SHC and East Bridgewater YMCA partner to open Healthy Living Center

Signature Healthcare and the Old Colony YMCA’s East Bridgewater Branch have collaborated to establish a Healthy Living Center at 635 Plymouth Street. The center will bring together medically integrated programs between Signature Healthcare and the YMCA with the goal of helping individuals achieve and maintain a healthy lifestyle.
A Signature Healthcare Health Coach will be positioned at the YMCA and will partner with YMCA members to encourage positive health choices and help members improve their overall health. The Health Coach will provide an initial assessment which will include a biometric screening. Upon the results of the initial assessment, the Health Coach will make recommendations for programs or services that will improve a patient’s health, such as programs at the YMCA or referrals to primary care providers or specialists at Signature Healthcare.
Members will routinely meet with the Health Coach and work on short term goals and behavior changes leading toward an improved state of health. The Health Coach will also occasionally provide reassessments to evaluate progress and determine future goals. The coaching partnership will continue for as long as 6 months to 1 year.

The national Y organization, YMCA of the USA, has seen other local organizations open Healthy Living Centers with area hospitals/health systems. Members of Signature Healthcare and the Old Colony YMCA had the opportunity to visit the partnership between YMCA of Florida’s First Coast and Baptist Health. Leaders from the organizations witnessed how the Healthy Living Centers were positively impacting the health of the local communities and are optimistic about bringing this program to the Greater Brockton area.
