Medical Records

There is no charge to send your records directly from Signature Healthcare to another provider, but if you wish to obtain a personal copy there will be a charge. Once the record has been retrieved, the Medical Records Department will provide a quote of what it would cost for the record and you will then have the opportunity to choose to go forward with the request and pay the charge or cancel your request.

All requests must be sent via the correct form, we cannot accept requests via email or phone. Please contact the Health Information Management Department (HIM) with questions:
